You’ve just gotten lip filler, you look in the mirror, and your lips are a lot more swollen than you expected. For many patients, that’s when the questions start. Is this normal? Are they going to stay this big? Why does one side look more swollen than the other?
Some swelling, tenderness, redness, and bruising can be completely normal after lip filler. The tricky part is that your lips can change quite a bit during the first several days, so judging your results too soon can create unnecessary worry.
Here’s a general idea of what the lip filler healing process can look like from the day of your appointment through the point when your lips begin to settle.
Why Do Lips Swell After Filler?
Your lips are a sensitive area with a strong blood supply, and lip filler requires a series of injections to place the product where it’s needed. Those injections create a temporary inflammatory response, which is why some swelling afterward is expected.
Many commonly used lip fillers are also made with hyaluronic acid, a substance that attracts and retains moisture. This can contribute to some of the fullness you notice shortly after treatment.
How much you swell is highly individual. One person may have relatively little swelling, while another may notice a much more dramatic change for the first few days.
Several factors can influence your response, including:
- How your body responds to injections
- The amount and type of filler used
- Your provider’s injection technique
- The specific areas of the lips treated
- Your natural tendency to swell or bruise
This is also why comparing your recovery to a friend’s experience or someone’s photos online isn’t always helpful. There’s a fairly wide range of what the healing process can look like.
Day 0: Right After Your Lip Filler Appointment
Swelling can begin fairly quickly after your injections. By the time you leave your appointment, your lips may already look fuller than you anticipated.
You may also notice:
- Swelling
- Tenderness
- Redness around the injection sites
- Minor bruising
- Tightness or firmness
- Some temporary unevenness
At this point, you’re seeing both the filler and your body’s immediate response to the injections. That makes Day 0 a poor time to evaluate the final size, shape, or symmetry of your lips.
Even if you love what you see right away, your lips are still going to change as they heal. And if they look a little bigger or less balanced than you pictured, give them some time before worrying about the outcome.
Days 1-2: Swelling May Be More Noticeable
The first day or two can catch new lip filler patients off guard because swelling doesn’t always disappear right away. In fact, it may become more noticeable before it starts getting better.
Your lips may look larger than they did immediately after treatment, and one side may appear slightly fuller than the other. Bruising can also become easier to see during this stage.
You might notice:
- More noticeable fullness
- Slight differences between one side and the other
- Tenderness when touching your lips
- More visible bruising
- Temporary firmness
All of that swelling can temporarily change the proportions and shape of your lips. An area that seems too full or uneven right now may look very different once the inflammation comes down.
As tempting as it may be to press on an area or try to smooth something out yourself, don’t massage or manipulate your lips unless your injector has specifically instructed you to do so.
Days 3-4: Swelling Often Begins to Settle
Around the third or fourth day, many patients start to notice that the initial swelling and tenderness are improving.
Your lips may begin looking more balanced as the inflammation decreases, although some puffiness can still be present. If you bruised, those areas may also change color as they heal.
This is often the point when patients start feeling more comfortable with how their lips look, but there’s no need to worry if you still have some swelling. Healing doesn’t happen on an exact schedule.
Days 5-7: Your Lips May Start Looking More Settled
By the end of the first week, much of the initial swelling may have improved. Your lips may feel softer, and their shape and definition should be easier to see without as much inflammation getting in the way.
Any bruising you experienced may also be fading by this point.
This stage usually gives you a much better sense of where your results are heading, but it’s still possible to have subtle swelling or small changes as your lips continue to heal. Your lips are still settling, so small differences at this stage may continue to change.
Days 7-10: A Better Look at Your Results

By this point:
- Most of the initial swelling should have settled
- Your lips should look and feel more natural
- Unevenness caused by swelling should be improving
- Your new shape, volume, and definition should be easier to evaluate
This is a general timeframe, not a deadline. Your body may settle a little faster or take a little longer, and that doesn’t necessarily mean anything is wrong.
Around Two Weeks: Time to Assess Your Results
Once you reach the two-week mark, you’ve given the initial swelling more time to resolve and the filler more time to settle. That makes this a much more useful point for evaluating your results than the first few days after treatment.

What Can Help With Lip Filler Swelling?
Your injector’s aftercare instructions should always be your first reference because recommendations can vary depending on your treatment.
Depending on your provider’s guidance, you may be advised to:
- Use a cold compress or ice pack as directed
- Avoid unnecessary touching, rubbing, or pressure
- Follow any temporary activity restrictions
- Give your lips time to heal before evaluating the result
Avoid pressing, aggressively massaging, or manipulating your lips unless your provider has specifically instructed you to.
What’s Normal vs. When Should You Call Your Provider?
Knowing that swelling is expected can make the first few days easier, but that doesn’t mean you should ignore something that feels unusual.
Usually Normal During Early Healing
Mild to moderate swelling, tenderness, redness around the injection sites, bruising, firmness, and some temporary unevenness can all happen during the early stages of healing.
Rather than expecting your lips to look perfect by a particular day, pay attention to the overall trend. In a typical recovery, these changes should gradually become less noticeable as your lips heal.
When to Check In
- Swelling is significant or continues getting worse instead of improving
- Pain is severe or increasing
- You notice unusual or concerning changes in the treated area
- Your symptoms aren’t improving as expected
- You’re simply unsure whether what you’re experiencing is normal
You don’t need to figure out on your own whether something is a complication. If something doesn’t feel right, contact your injector. They can evaluate what’s happening and determine whether you need to be seen.
Give Your Lip Filler Time to Settle
The first few days after lip filler can be a little misleading. Swelling can temporarily make your lips look larger, firmer, or less symmetrical than the result you’ll ultimately see.
For most patients, the best approach is to follow their aftercare instructions and give their lips time to heal before deciding how they feel about the outcome.
